Only the original 2-part form needs to be turned in. Additional copies are not necessary.

  • You must indicate the number of registrations and amount of dues and if any donations
    are being made.
  • You must add in the leaders name, address, phone or leader ID #, if known, at the bottom.

Note: If you use this fill-in form, please make (3) copies

  • Please make sure that all information is completed by the parent including name, address, phone number, registration year, etc. Be sure parent signs the form
  • PLEASE MAKE SURE I HAVE BOTH COPIES OF THE FORM.
    If the parent keeps one of the copies by mistake, you can submit a photo copy with the registration packet. If the
    troop wants to keep a copy, you will need to make photocopies for your records and submit the original 2-part form with the troop registration packet.
  • Racial and ethnic information is optional from the parent. If it is not completed by the parent, the leader or troop registrar should complete the information and indicate one or both was completed by the leader by checking the visual survey indicated in the lower right of the form in the colored margin as "VS 1" for racial data and "VS 2" for ethnic data.
  • The following codes need to be added in the boxed area at the lower right in the colored margin of the page: Fill in the corrent codes for your Service Unit from the table on the below.

  • Important: If the adult holds multiple positions in one or more troop, please indicate EVERY position held and for EVERY troop at the top of the form.
    NOTE: There is only one '01' per troop. Co-leaders are '02' and all other troop positions (i.e. treasure, cookie chair, responsible adult, etc.) are '03' position codes

Step 3

Payment by Troop Check
  • Please submit a troop check for the total amount of the registrations only with 2 signatures.
  • Cost of registration is $12.00 per person (Scout or Adult)
  • Make check out to GSCOC -YLSU_(your SU#) ex: GSCOC YLSU3
  • DAISY TROOPS NOTE: Daisy troops may submit parent checks or cash for registration which includes Daisy troops that are submitting early renewal registration before June 15 even if they will be Brownies in the fall.
  • Please note that the registrar is not able to provide receipts for cash payments from parents.
  • It would be appreciated if Daisy troops could place all the deposit checks and cash in one envelope sealed with the total amount noted on the outside of the envelope.
  • If a donation is being submitted with the registration, please submit a separate check for the donation. It can be a troop check (with 2 signatures of course) OR a parent check for the donation. Either is acceptable as long as it is separate from the registration check. This may seem petty, but the donation and registration checks go to different
    council accounts so it's a must!

Step 4

Bank Authority Form
  • All levels (except Daisy) must have a Bank Authority Form on file with all authorized signatures
  • After 2005/2006 this form will only need to submitted if the signature authorities changed from the prior year.
  • The Bank Authority form on file must include all current authorized signers. If any signers were added or deleted from the Troop Account, please submit a new Bank Authority form to the Service Unit registrar.
  • Daisy troops participating in early registration that will be Brownies in the fall do not have to provide this document for early registration before June 15, however, it will need to be submitted no later than October 1st
