Bank Authority Form

All levels (except Daisy) must have a Bank Authority Form on file with all authorized signatures
After 2005/2006 this form will only need to submitted if the signature authorities changed from the prior year.
The Bank Authority form on file must include all current authorized signers. If any signers were added or deleted from the Troop Account, please submit a new Bank Authority form to the Service Unit registrar.
Daisy troops participating in early registration that will be Brownies in the fall do not have to provide this document for early registration before June 15, however, it will need to be submitted no later than October 1st
